            Medication Supplies


            Prescription medications and diabetic supplies are coordinated by accessing Patient Assistance Programs (PAP's) of the pharmaceutical companies.

            How Does the Program Work?
            • The Clinic assists clients in filling out applications for free prescription medications.
            • When applications are approved, the medications will be delivered to the client's home or physician's office, to be picked up by the client.

            How Do I Apply?
            • Call (616) 392-3610.  A telephone intake assessment will be scheduled to determine eligibility.  This call takes approximately 15 minutes. 
            • If eligible, a scheduled appointment will be made in which the client brings in all required documentation and the initial applications and forms are filled out and signed.  At that time, if all information is complete, the client will be enrolled and the process to obtain medications will be started. 



            How Long Does It Take to Get Medication?
            When accepted and enrolled into the program, it will take 4-6 weeks to receive your first shipments of medications.

            What Information Will I Need to Supply?

            • Most recent 30 days of documented income.
            • Latest bank statement for all accounts.
            • Federal Tax Return
            • Copy of all insurance cards.
            • Medicaid denial or spend down letter.
            • Medicare Part D subsidy denial or approval letter.
